Have you had your flu jab yet?  We are running a walk-in clinic on Saturday 7th October from 8.30 - 11.00am. No need to book an appointment - just turn up!

Our new telephone queuing system will be operational on Wednesday 11th October.  We hope this will help patients to contact the surgery more easily.

Patients registered to GP practices in North Hertfordshire are being asked their views on GP opening hours by East and North Hertfordshire Clinical Commissioning Group (CCG) and NHS England. The survey can be found here

Have you signed up for online access yet?  It is the easy way to order repeat prescriptions and you can also book and cancel appointments online.  Download the application form using the link at the bottom of the page.

Pre-Registration with the Surgery

PreRegistrationIf you wish to pre-register click on the link below to open the form. When you have completed all of the details, click on the "Send" button to mail your form to us.  We cannot register you without your NHS number, so please ensure this is included. 

Registration will not be completed until you have presented photographic evidence of identity and confirmation of your address (e.g. passport and utility bill not more than three months old) at the practice.  All patients are asked for these documents.  You will be unable to make appointments until registration is complete and your details are on our computer system.  If we have any queries with your details we will endeavour to contact you.

When you register you will also be asked to fill out a medical questionnaire. This is because it can take a considerable time for us to receive your medical records. There is an online version of this file too, which you may fill out and send to us. You will only be asked to come to surgery to sign this form if you opt to join the organ donor register.

Alternatively you may print off a registration form, fill it out and bring it in with you on your first visit to the practice.
